The Office of the Child and Youth Advocate has completed its Investigative Review involving the death of a 14-day old baby girl and is publicly releasing the results of the review as outlined under the Child and Youth Advocate Act.
“The circumstances around this infant’s death are both tragic and troubling,” said Del Graff, Provincial Child and Youth Advocate. “What is particularly disconcerting is that despite an abundance of good information, the system appeared unable to come together for the benefit of this child.”
According to Graff, getting an accurate impression of a child requires information from multiple sources. “Regretfully, in this child’s circumstance, although there were many professionals involved, valuable information was not shared among them. The system needs to work better for vulnerable children.”
